OFFICE APPOINTMENTS POLICY AND COVID-19 PROTOCOL

Mask are required during your entire appointment.

This office will continue to follow COVID protocol.

Please re-schedule your appointment if you are sick or if you have been around anyone that is not feeling well. We are staggering appointments and cleaning touched surfaces between all clients. Please do not come more than 10 minutes before your scheduled session. Please contact Annetta for appointments. Text or call 845-443-1280.

When I am in a session, I am unable to answer all calls. Please text or leave a message and I will respond as soon as possible.

I ask that you try to book your appointments with as much notice as possible.

Appointments may be available outside of the above listed hours (subject to availability), but requires an AFTER HOURS SURCHARGE of $50 for office appointments.

If you would like to make an appointment outside of the allotted times available, please reach out to me prior to booking.

POLICIES

Payment is expected BEFORE treatment begins to help expedite the checkout process. You may leave a tip before or after your service.
Choose Massage LLC has a 24-hour cancellation policy. If you cancel your appointment 24 hours or less before your appointment or if you do not show up for your appointment without advanced notice, you will be charged the full amount.
All appointments are to be confirmed by Annetta Childs before final booking is confirmed. You will receive a confirmation once approved.
Please arrive 10-15 minutes early for appointment to allow for introduction, assessment and/or necessary paperwork.
For late appointments, I will do my best to accommodate your session duration without affecting the sessions of clients booked after you.
I require credit card information to hold your appointment time. I suggest booking your appointment at least a week in advance as I can often book up very quickly.
Please arrive on time for your appointment. If late arrival is inevitable, your service(s) will be shortened in order to keep on schedule. No full or partial refunds will be given.
Inappropriate interaction or discussion of any kind between the client and the massage therapist is NEVER appropriate. Thank you for your understanding and cooperation.
Hold your Gift Cards securely. Gift Cards that are lost or stolen cannot be replaced and Choose Massage LLC shall not be responsible for lost or stolen cards.
Gift Cards may only be redeemed for products or services.
Gift Cards cannot be redeemed for cash or credit, except To the extent required by law. Gift Cards may not be used to pay for a package. Gift card sales are final and nonrefundable. Gift cards cannot be combined with discounts or promotions. Prices are subject to change.

TIME LIMITS FOR SERVICE

Specials, Flash Sales & Promotional Priced Services MUST BE completed within 30 days of purchase date.

Regular Price Services MUST BE completed within 60 days of purchase date.

Packages MUST BE used within 90 days of purchase date.
